The traditional practice of canine tooth bud removal in children and its impact in the developing dentition in north west Ethiopia

Objectives: The aim of the study is to determine the prevalence of canine tooth bud removal,cause and its complication in the erupting dentition among children visiting dental clinic of university of Gondar hospital.
Methods: An institution based cross sectional study was conducted from March2015- March 2016 at university of Gondar hospital dental clinic. Three hundred fifty five children within 2-12 years old were evaluated for oral mutilation and their families/guardians were interviewed. The status of each canine was noted to determine the presence of missing deciduous canine and developmental dental defects. For testing of significant, catagorical data was compared using chi-square test and bivariate and multivariate analysis was done to assess the asoociation of independendet variables with the dependent variable.
Results: A group of 355 children with a mean age of the children was 7.32±3.12 (SD) were involved. The prevalence of canine tooth bud removal was 81.9%. Maternal age and educational level of the mother had significance impact on the occurance of canine tooth bud removal (AOR=0.378, 95%CI: 0.226-0.633;AOR= 3.811, 95%CI: 2.323-6.246 respectively).
The most common dental complications were; malformed enamel (hypo plastic) canine (48.5%) and missed/Unerupted canine (38.6%). 99.15% of the practice is found in the mandibular canine. There exists a strong belief that diarrhea, vomiting and febrile illness of the child are due to the swelling on the canine tooth bud area.
Conclusions:
The prevalence of canine tooth bud removal is high. Canine tooth bud removal is more prevalent in the study area. Majority of the child had malformed canine. So, a multidisciplinary approach is recommended to discourage this harmful practice.These findings show an urgent need to have community-based campaign, to build their knowledge on the complications and effect of canine tooth bud removal on the quality of life in children. A culturally sensitive training should be given to change the social belief of the practice.
