Salivary Creatinine as a Diagnostic Tool for Evaluating Patients with Chronic Kidney Disease.

Objectives: To correlate the serum and salivary creatinine levels at each stage of chronic kidney disease (CKD) and and evaluate the role of saliva as a safe and non-invasive alternative to serum for creatinine estimation.
Methods: A cross sectional study was conducted at the Renal Unit at Tygerberg hospital on 230 patients at all stages of CKD. Informed consent was obtained, whereafter saliva and serum samples were collected and analysed for creatinine levels. Correlation between plasma and salivary creatinine was determined using Pearson’s correlation test. Receiver operating characteristics (ROC) analysis was done to determine the diagnostic ability of salivary creatinine and cut-off values were obtained.
Results: Serum creatinine values for all patients ranged from 0.52mg/dl to 17.89mg/dL with a mean of 2.52mg/dL (SD 2.591). Salivary creatinine values ranged from 0.03 to 4.52mg/dL with a mean of 0.343 (SD 0.576). Pearson correlation analysis showed a positive correlation between serum and salivary creatinine at all CKD stages. Linear regression analysis of serum and salivary creatinine for CKD patients was significant in all CKD stages except for stage 1. Area under the curve for salivary creatinine was found to be 0.859. A cut-off value of 0.1mg/dL gave a sensitivity of 77.2% and specificity of 80.0%.
Conclusions: The results of this study support the diagnostic potential of salivary creatinine as a non-invasive diagnostic tool for estimating serum creatinine in chronic kidney disease patients.
