Impact of Malocclusion among Adolescents on Family Quality of Life

Objectives: To assess the prevalence of malocclusion and evaluate its impact on the family's Quality of Life (QoL) using the Family Impact Scale (FIS) across the different levels of Socio Economic Status (SES).
Methods: A cross-sectional study was conducted among 12- 15 year old schoolchildren and their parents in Udupi taluk, Karnataka,India. Parents of 768 children filled the Kannada version of the FIS questionnaire.The prevalence of malocclusion was evaluted using Dental Aesthetic Index(DAI) among schoolchildren by a single caliberated examiner. The socioeconomic status was assessed using Kuppuswamy scale.Malocclusion was dichotomised as absent (DAI < 25) and present ( DAI≤ 25) for statistical analysis.Descriptive analysis was used to calculate the frequencies of various socio-demographic variables of the children. The Mann– Whitney test was used to assess the significant differences in the subscales and overall FIS scores between parents of children with and without malocclusion.
Results: Out of the 768 children 59.9% (n=460) of the children had malocclusion while 41.1% (n=308) had no malocclusion. Among the children with malocclusion, 30.7% (n= 141) were <13 year old and 69.3% (n =319) were >13 year old, which included 53.0% (n= 244) males. The prevalence of malocclusion among the children was 26.5% (n=122), 38.5% (n=177) and 35% (n= 161) in the upper, middle and lower SES respectively.On comparison there was no significant difference in the prevalence of malocclusion between age (p= 0.143), gender (p=0.956) but SES (p=0.023). The mean scores of overall FIS and subscale were significantly higher among the parents of children with malocclusion(p<0.001). Further, when compared parents of children who had malocclusion had reported significantly higher mean overall FIS scores than parents of children without malocclusion in the upper, middle and lower strata (p<0.001).
Conclusions: The presence of malocclusion had a negative impact on the overall well-being and QoL of the family in the different SES.
