Surgical Periodontal Therapies: A Retrospective Study ON 2729 Patients

The periodontium may serve as a reservoir of bacteria, bacterial products, and inflammatory and immune mediators which can interact with other organ systems remote from the oral cavity. Periodontal infections may increase the risk for certain conditions by contributing to disease pathogenesis or by serving as a source of infective organisms.

Objectives: the aim of our study is to compare the data obtained from a traditional surgical approach and the one got from the use of autologous PRP (Platelet-rich plasma).

Materials and methods: 3181 patients had been visited from 2000 to 2008 at the Periodontal Department of the Milano-Bicocca University's Dental Clinic (S. Gerardo Hospital). 2729 patients were treated with traditional surgical tips (control group): 704 were aged between 16 and 30 (young patients), 972 between 31 and 64 (adults) and 1053 over 65 (elderlies). According to the Lindhe classification, patients were divided in different groups: subjects with slight periodontitis (1092), subjects with severe periodontitis (1673). 452 patients were treated with surgical tip associated with PRP (test group). 121 were young, 183 adults and 148 elderlies. 105 subjects showed slight periodontitis and 337 severe periodontitis. The probe depth was evaluated through manual periodontal probes at the initial diagnosis, after the Initial Causal Therapy, 3 and 6 months after the periodontal surgical treatment. T Student test was used for the statistical evaluation of the protocol efficacy (p<0.05).

Results: results show a statistically significant reduction in probe depth in patients with severe periodontitis, adults and elderlies, while there were no statistically significant differences in the other categories of patients.

Conclusions: basing on the obtained results, considering all the limits of the research, the surgical treatment (tip) associated with PRP, is a method which guarantees valid and predictable results for the resolution of the severe periodontal disease in adults-elderly patients
