Analisys of Cardio-circulatory Changes During Dental Surgery in Pre-hipertensives Patients

Objectives:The aim of this study was to evaluate cardiovascular changes during bone graft surgery using 2% mepivacaine with 1:100.000 epinephrine. Methods: Twenty three pre-hipertensives subjects, ranging from 18 to 56 years, were selected to undergo bone graft surgery in the upper jaw. They were monitored in the pre-, intra-, and postsurgical periods by continuous noninvasive automatic arterial pressure and cardiac frequency measurements taken every 2 minutes. Parameter scores were obtained for the following phases: (S1) 15 minutes during preparation of the patient (control period); (S2) before anesthesia; (S3) immediately after anesthesia; (S4) 2 minutes into anesthesia; (S5) during the surgery procedure; (S6) during suturing; (S7) on completion; and (S8) 10 minutes after termination. The anesthetic efficacy was checked by visual analog scale for intra-operative pain. Individualized statistical analysis for each group during the pre-, intra-, and postoperative periods were performed by Analysis of Variance (ANOVA) and the t test (p< .01). Results: The greatest variations in systolic pressure were increases of 2.23% during phase S2 and 2.98% in phase S4. Diastolic pressure decreased during phase S4 (-5%), S5 (-5.12%) and S6 (-5%). The Mean Arterial Pressure decreased 7,37% during Phase F7. The second best change in Mean Arterial Pressure was during Phase F6 (-6.26%). The greatest changes in heart rate occurred in phase S3 (6.2%). Conclusions: There were no statistically significant changes among the evaluated phases (P>.01). The medium dose of mepivacaine administered was 144 mg and of epinephrine was 72.36 µg. In conclusion, there were no changes in the analyzed cardiocirculatory parameters during bone graft surgery in pre-hipertensive subjects anesthetized with 2%mepivacaine with epinephrine 1:100.000.
