Management of Severely Traumatized Permanent Teeth: Case Series

Objectives: Traumatic dental injuries (TDIs) are frequently reported after dental caries. The objective of the study was to report the management of severe TDIs affecting permanent teeth of five patients.
Methods: All patients undergone clinical examination including, demographic data, medical history, extraoral and intraoral investigations and radiographic explorations using retroalveolar radiographs and cone beam computed tomography (CBCT).
Results: The patients were two females and three males. Their age varied from 9 years to 22 years old. All TDIs were associated to alveolar bone fracture. At least two teeth were involved in two patients. In three patients, the number of involved teeth varied from three to six teeth. Avulsion of central maxillary incisors were recorded in two patients. Luxations of maxillary and lateral incisors were noted in two patients. In one patient TDI involved six teeth with uncomplicated crown fracture (one mandibular lateral incisor), complicated crown fractures in three teeth (maxillary central incisor and mandibular lateral incisor and canine) and avulsion of three teeth (maxillary central and lateral incisor and canine). Radiographic investigations were performed to eliminate forreign bodies in case of avulsed teeth or to detect other types of fractures associated to traumatized teeth, such as rroot fracture. All interventions were performed under local anesthesia. Lesions to soft tissues were sutured. fractured alveolar bone fragments and displaced teeth were surgically repositionned (2 patients). Avulsed teeth were replanted. Endodontic treatments were performed after teeth splinting. The outcome was favorable in 4 patients with a follow-up period extending from 2 years to 17 years. In one young patient, inflammatory resorptions led to teeth decoronation.
Conclusions: Management of severe TDIs of the permanent teeth may be complex and challenging. The dentist should conduct an accurate exam to ensure a favorable outcome, according to the International Association of Dental Trauma.
