Objectives: To report two cases of flare ups post endodontic treatment and to identify potential causes and therapeutic procedures to adapt through a literature review. Methods: Patients’ history and clinical examinations were performed. Also, a literature review on flare-ups was conducted using PubMed and Google Scholar using appropriate keywords and criteria. Results: A 16-year-old female consulted due to a past trauma on tooth 41. The patient reported previous pain associated with the tooth. Clinical and radiological examination revealed chronic apical periodontitis with a latero-apical radiolucency. Endodontic treatment was performed. A day post-treatment, the patient reported pain and swelling , with no signs of lymphadenopathy or fever. Analgesic treatment was prescribed and three days later, a regression of clinical signs was noted, with lesion resolution observed by the end of the week. A 43-year-old male consulted for endodontic retreatment of tooth 23. Clinical and radiological examination revealed the tooth to be asymptomatic, displaying a periapical image and early signs of apical external resorption. A session for previous obturation removal and medication was performed, followed by canal obturation 14 days later. The patient returned after two days with complaints of pain, high upper right jaw swelling, lymphadenopathy, and fever. Antibiotic therapy and analgesics were prescribed for a week. Symptomatic regression was observed after 2 days, and lesion resolution was noted one week later. Different studies from the literature showed that flare-ups represent a multifactorial phenomenon including mechanical, chemical and microbial factors. In addition, a correlation was found between flare-up and age, gender, tooth type, presence of preoperative pain, tooth condition before treatment, irrigation techniques, number of visits as well as intracanal medication. Conclusions: Flare-ups are multifactorial; it's challenging to fix a clear treatment strategy.
