Gingival Fibromatosis ; Case Reports

Objectives: This poster aims to present the clinical aspect of Gingival fibromatosis, to highlight the management and the challenges that this pathology could impose
Methods: Three patients aged respectively 10 ,9 and 11 from which two are siblings, were clinically diagnosed with gingival fibromatosis and later confirmed by histological exams. All patients presented excessive gingival growth covering in places most of the crowns causing esthetic and psychological prejudice. Radiological exams showed several permanent teeth were blocked.
The histological exam revealed an important density of type1 collagen fiber in the extracellular matrix confirming the diagnosis of gingival fibromatosis
The familial aspect was found in the two siblings as for the other patient the excessive gingival was an isolated case.
Both patients received initial periodontal therapy and oral hygiene motivation to prepare for the surgery. Later multiple gingivectomies were performed, and several temporary teeth were extracted to allow permanent teeth to erupt
During all stages of treatments both patients were constantly reminded to maintain a good oral hygiene.
Results: After the initial and surgical treatments, all cases resulted in an acceptable outcome. All patients showed significantly less gingival tissues while smiling, the permanent teeth started erupting after being blocked. Moreover, all patients were very satisfied with treatment, showing an improved psychological behavior and an increased self-esteem
Nevertheless the lack of home care by the patients was the main obstacle towards an optimal result, in addition two of those patients presented a buccal respiration which favors the accumulation of plaque making the inflammation a gingival growth persist.
Conclusions: Patients with fibromatosis require thorough examination in order to adapt the treatment. Resective surgery of the excess tissue is the usual treatment.
However, the patients and parents should be made aware that recurrence is a common feature.
