Cemento-Ossifying Fibroma of the Mandible: Conservative Management Versus Interruptive Resection.

Objectives: Cemento-ossifying fibromas (COF) are rare fibro-osseous neoplasms that affect the jaws. They are included in the benign mesenchymal odontogenic tumors.
This work aims to provide the clinical and radiographic features of two cases of COF and to detail their management.
Methods: Two patients who consulted the dental medicine department of Fattouma Bourguiba university hospital.
Case 1:
A 30-year-old male reported with a complain of a painless, slow growing left mandibular swelling for 3 months.
Extra-oral examination showed a diffuse swelling over the left body of the mandible. It was present in the buccal vestibule and has been extended into the molar region.
Panoramic radiograph showed a well-defined unilocular polycyclic radiolucency, and extending the inferior left border of the mandible.
CT images revealed a large well- defined mixed lesion containing central calcifications, in the left side of the body of the mandible. Buccal, lingual and inferior plate of the mandible were thinned and interrupted.
The patient was referred to the Maxillofacial Surgery Department for surgical management under general anesthesia. The course of action was an interrupted resection of the mandible associated with a reconstruction by an iliac graft.
Histological exam concluded the diagnosis of COF.
Case 2:
A 26-year-old female reported for an intraoral buccal swelling evolving for one month.
Panoramic radiograph and CBCT revealed a well-defined multilocular radiolucent lesion in the right mandibular region with internal diffuse scattered radiopacity. Surgical enucleation with curettage of lesion were done under local anesthesia and sent for histopathological examination that concluded the diagnosis of COF.
Results: Good mucosa and bone healing was obtained for both cases. No recurrence was observed after one year and six months for case 1 and case 2, respectively.
Conclusions: The treatment of COF must be directed towards the complete removal of the tumour mass using enucleation or curettage techniques. Surgical resection of the jaw is recommended only for large neoplasms.
