An Update on Pulpotomy Techniques in Primary Teeth : a Systematic Review of the Literature
Objectives: Controversies surround the issue of pulpotomy agents, and the “ideal” pulp dressing material that can replace Formocresol (FC) has not yet been identified. Aim: the aim of the present systematic review was to compare modern pulpotomy techniques in order to establish a suitable alternative to formocresol pulpotomy. Methods: A broad search of Medline, Cochrane, Scopus, Science Direct and EBSCOhost databases was conducted using (MeSH) terms. Relevant papers published in English and French were identified after a review of their titles, abstracts and full texts. Results: Out of 484 articles identified in initial research, 14 articles meeting the inclusion criteria were included. Based on this review results, although (MTA) showed higher clinical and radiological success rates, the treatment success with all the tested materials was similar to the (FC) pulpotomy reaching non statistically significant difference at all follow-up times. Conclusions: The studies included in the present review did not categorize the tested pulpotomy techniques into hemostatic agents and pulp capping materials, which did not allow to draw concise results. Hence, a general consensus regarding the most appropriate pulpotomy technique remains challenging. Although, promising results were provided by Biodentine™, Calcium-enriched mixture, Portland Cement and Enamel Matrix Derivative. Further randomized clinical trials (RCTs) with standardized management procedures and outcome measures, larger samples and above all longer follow-up times are needed to support these outcomes.
