Acute Severe sub Mandibular Adenophlegmon in a Child With mal Controlled Type 1 Diabetes: a Life Threatening Emergency

Objectives: Diabetes is a risk factor for severe infectious complications, especially when it is poorly managed or mal controlled.
Diabetes management in children can be particularly difficult, especially when the family is not efficiently involved. The child is therefore not very observant and complications can arise at any time.
Pulp necrosis and their locoregional complications such as, cellulitis, can present life threatening complication when the infection spreads regionally or systematically.
This work aims to report the case of a severe complication of pulp necrosis in a child with type 1 diabete.
Methods: A10-year-old patient with TYPE 1 diabetes diagnosed at the age of 5y-old, treated with insulin,
was referred by his attending doctor for a severe left submandibular swelling evolving for 3 days in a febrile and painful context. The patient had poor oral hygiene and decayed molars on the homolateral mandible.
Glucosylated hemoglobin was 10%.
A panoramic X-ray as well as an echography were performed.
Echography showed a 10mm hypertrophied jugulocarotid adenomegaly with multiple reactive bilateral
cervical lymph nodes
The diagnosis of acute diffused cellulitis complicated with submandibular adenophlegmon was perfomed.
Results: Intravenous antibiotic therapy in addition to surgical drainage of the collection were performed.
Conclusions: The spread of dental infection is a potentially life-threatening emergency, especially when it occurs in a sick patient.
In children, diabetes is particularly difficult to balance and specially to gain the compliance of the child and his family. Motivation, information and follow-up is a winning triad to keep in mind when dealing with this type of patient.
Once the diagnosis is established, management must be carried out urgently, in an appropriate environment and ensuring follow-up at least in the medium term.
