Diode Laser Efficacy in the Treatment of Oral Lichen Planus : a Systematic Review.

Objectives: The treatment of oral lichen planus (OLP) remains a great challenge for clinicians; various drugs have been used with dissimilar results. Topical corticosteroids are commonly used as standard therapy. However, patients frequently present relapses after the discontinuation of treatment as well as developing resistance to corticosteroid therapy. Recently, laser has been suggested as a new treatment option for OLP.
The aim of this systematic review was to detail diode laser approaches used for the treatment of OLP.
Methods: We conducted a comprehensive electronic search using the electronic databases available in MEDLINE/ PubMed, until June 2020, using combination of the following keywords: «oral lichen planus», «therapy» and «laser».
The focused question of interest was «what are the diode laser approaches used for the treatment of OLP? ».
Inclusion and Exclusion criteria were taken into consideration for the selected publications.
Results: A total of 110 publications were initially identified, 17 non-English articles were then excluded. After selection, only 13 studies were included in this systematic review. Thediode laser was used at a low level and so nominated low level laser therapy (LLLT) in all included studies. Four studies compared the efficacy of diode laser with corticosteroids; one study compared LLLT to three therapies: ozone, corticosteroid, and a placebo; one study compared LLLT to two therapies: corticosteroid and photodynamic therapy. One study compared the efficacy of the LLLT to CO2 laser surgery. Others studies were case series. All studies assessed the effect of LLLT on pain alleviation and clinical improvement in patients with OLP.
Conclusions: A wide range of parameters and treatment protocols as well as outcomes were observed, and no effective protocol could be established.
