Surgical Management of Minor Salivary Glands Pleomorphic Adenoma

Objectives: Salivary gland tumors represent around three percent of the head and neck tumors. Pleomorphic adenoma is the most frequent benign tumor of the salivary glands. In most cases, it arises from the parotid and submandibular salivary glands. Rarely, it affects the minor salivary glands located throughout the oral mucosa.
The purpose of this presentation was to discuss, via a clinical case of a pleomorphic adenoma of the hard palate treated by surgical excision, the clinical and histopathologic characteristics, and to review the literature on pleomorphic adenoma of the hard palate while specifying surgical details preventing recurrence.
Methods: 56 year old female presented with complaints of a painless, slow-growing mass of the hard palate.
The examination showed an ovoid-shaped swelling located in the left hard palate, asymptomatic, sessile with a firm consistency, measuring 3 cm, the overlying mucosa was non-ulcerated. A computed tomography scan showed no locally infiltrative feature.
The provisional diagnosis was a benign salivary gland tumor and surgery was planned.
Results: The treatment was surgical excision.
Histopathological examination confirmed the diagnosis of pleomorphic adenoma.
Conclusions: The reported case was consistent with the literature in terms of epidemiology, clinical and histopathological presentation. Pleomorphic adenoma presents a predilection for the female sex and for the age from 30 to 60 years. On minor salivary glands, the most frequent site is the hard palate region.
The diagnosis of pleomorphic adenoma is established on the basis of history, clinical and histopathological examination showing mixed epithelial cells and mesenchymal elements. CT-scan is necessary to determine the extension of the lesion.
Complete surgical excision with negative margins is the treatment of choice.
Periodic follow-up is recommended because of the potential risk of recurrence and malignant transformation.
