Dentin Surface Pretreatment Optimize the Bond Strength of Composite Resin

Objectives: The durability of the bond between dentin and resinous adhesives is currently an area of interest in restorative dentistry. The purpose of this study was to determine whether using surface pretreatment agents has a detrimental effect on bond strength of the bonded assemblies. The effect of three agents was tested with universal adhesive.
Methods: Human molars (n=80) was used in this study. The occlusal surfaces of the molar crowns were removed with a diamond disc to expose flat dentin surfaces. The teeth were divided into 4 groups (n=20) according to pretreatment agent NaOCl 3.75% , DMSO 0.5 mM, CHX 2% and control group). Each group was subdivided into two subgroups of 10 according to the adhesive technique (Self Etch (SE) or Etch and Rinse (ER)). Shear Bond Strength (SBS) was calculated from resistance measured using the H5KS Model HTN-5000N universal tensile machine. The data were analyzed using univariate analysis of variance and Fisher LSD tests (alpha=0.05).
Results: The strongest bonding interface was found for the NaOCl group SE (SBS =18.46) followed by the DMSO group ER (SBS =10.81). For SE groups, the SBS varies significantly (p < 0.05) except between the DMSO group and CHX group (p=0.52) and between the control group and CHX group (p=0.09). For ER groups, only DMSO and NaOCl groups recorded significant variations compared to the control group (p < 0.05).
Conclusions: The use of a surface pretreatment before bonding the composite resins improves the adhesion of a universal adhesive with or without prior etching. To improve the bond strengths to dentin, the use of a universal adhesive in the SE technique on a surface pretreated with ClONa at 3.75% is recommended.
