Endodontic surgery: a case report

Objectives: To describe protocole of endodntic surgery
Methods: A 55-year-old male consulted at the department of endodontics and conservative dentistry of the University Dental Clinic of Monastir colleague for treatment of the right lateral incisor (The chef complain was a spontaneous pains in the antero-superior sector for a few days associated with recurrent infectious episodes)
Oral examination showed a filling of the bottom of the vestibule painful to the palpation and which gives rise to pus. The 12 has significant abrasion and does not respond to the pulp sensitivity test. The Cone Beam radiographic examination revealed a radiological periapical image related to the 12 associated with bone dehiscence. This radiological aspect is very suggestive of an infected cystic lesion.
An endodontic treatment was conducted with Protaper Next. For the root canal filing, we used a biocermaic sealer ( Bioroot ,Septodent). Followed by a full thickness flap to access surgically to the lesion, which allowed its curettage and elimination. The tissues excised was addressed to an anatomopathological exam. Then, the apex of the root of the lateral incisor who was submerged in the cyst has been removed and retrograde obturation was conducted with MTA. Finally, the flap was sutured. An antibiotic, pain relief and a mouthwash was prescribed. The patient was seen after 10 days for removing the sutures.

Results: The cone beam is important before the surgery.
We should start by doing endodntic treatment to eliminate micro organism.
For our case endodontic surgery and apical resection is indicated.
We should have radiological control.
Conclusions: The endodontic treatment of the incisors with the surgical access and management of the lesion was sufficient to treat the radicular cyst
