Dental implant placement in diabetic patients: a systematic review of the literature

Objectives: to highlight the effects of diabetes on implant osseointegration and on infectious complications particularly Peri-implantitis.
to provide informations about precautions to take during and after dental implant surgery.
Methods: A review of the literature was done on the database "medline", via its interface "pubmed", using the following mesh terms: Diabetes Mellitus, Hyperglycemia, Dental Implants, Peri-Implantitis, Osseointegration.
Inclusion criteria:
case report articles, original articles, cohorts, randomized controlled trials, case-control studies, systematic reviews, meta-analyses
Non inclusion criteria:
review of narrative or guidelines-type literature, articles not published in English or French, articles dated more than 10 years, laboratory study
The boolean equations used were:
"Diabetes Mellitus"[Mesh] OR "Hyperglycemia"[Mesh]) AND "Dental Implants"[Mesh]
"Diabetes Mellitus"[Mesh] OR "Hyperglycemia"[Mesh]) AND "Peri-Implantitis"[Mesh]
"Diabetes Mellitus"[Mesh] OR "Hyperglycemia"[Mesh]) AND "Osseointegration"[Mesh]
Results: 91 articles were selected for lecture, 53 were excluded and 38 studies met the inclusion criteria.
The current literature supports dental implant placement in diabetic patients, no significant differences was noted in success and survival rates between diabetic and non diabetic patients.
The authors recommend antibiotic prophylaxis one day or one hour before dental implant placement and 7 days after, the use of Chlorhexidine® for mouth rinses.
Studies show that implant stability requires more time and peri-implant bone loss is more important in diabetic patients compared to non diabetic ones. hence, they recommend no immediate implant load.
Conclusions: The current literature supports dental implant placement in diabetic population with the need of some precautions, the maintain of good oral hygiene and regular dental controls .
More studies are needed to compare the results of dental implant placement in diabetic type 1 and type 2 separately.
