Isolated Lip involvement in oral lichen planus : A case series of 4 patients

Objectives: Oral lichen planus is a common chronic inflammatory disorder occurring mainly in the oral mucosa. Isolated lip involvement is rare. Our aim was to investigate clinical and histological features of lip lichen planus (LLP) to improve early diagnosis and malignant transformation detection.
Methods: Four cases of isolated lip lichen planus, with one degenerating into micro-invasive squamous cell carcinoma (SCC), were diagnosed and treated in our oral surgery unite of Farhat Hached , Sousse hospital.
Results: The clinical features consisted on erythema, erosion and mild keratosis involving the two lips in one case and only the lower lip in the other three cases. All cases were treated with Betamethasone application except for SCC which was managed by surgical excision.
Conclusions: Isolated LLP involvement diagnosis is challenging as it has not been well documented in literature. A long follow-up is mandatory for their malignant potential transformation and possible cutaneous-like progression.
