Oral discomfort related life quality in adult patient undergoing cytotoxic chemotherapy
Objectives: Chemotherapy is among the most indicated cancer treatment. Although it attacks quickly dividing cancer cells, it may cause many oral side effects that may affect patients life quality. This study aimed at assessing oral discomfort prevalence and oral health index related life quality.
Methods: A cross sectional study was conducted at the carcinology and hematology departments in the following Tunisian teaching hospitals: Farhat Hached in Sousse and Fattouma Bourguiba in Monastir during March and April 2018. A number of 150 adult patients undergoing cytotoxic chemotherapy were involved. We used as a measuring instrument a questionnaire that included an Oral health index score assessment (GOHAI SCORE). Student t test was used to compare GOHAI mean scores. Probabilities of less than 0.05 were considered as significant
Results: Thirty-seven male and 113 female patients were enrolled. The mean age was 50.36 (SD=10.1). 80% (n=120) of the patients had a low GOHAI score (< 50) associated with affected life quality. A statistically significant association were observed between low GOHAI scores and groups reporting mouth dryness (p<0.0001) and oral pain (p<0.0001). Low scores were statistically associated with alkylating (p=0.028), antimetabolites (p=0.029) and DNA cross-linking (p=0.01) agent administration.
Conclusions: Chemotherapy especially DNA cross-linking, antimetabolites and alkylating agents administration was associated with low oral health related life quality scores. Most reported oral symptoms were oral pain, oral dysgeusia and mouth dryness. Those symptoms may be related to several grades of drug-induced oral mucitis . Involvement of dentists and oral medicine doctors is mandatory to establish preventive and therapeutic strategy
