A rare lesion of the buccal mucosa: Giant Cell Fibroma

Objectives: Giant cell fibroma (GCF) is a new entity that belongs to the group of fibrous hyperplastic lesions of the oral mucosa; first described in 1974 by Weathers and Callihan. GCF accounts for 2-5% of all fibrotic lesions. According to our knowledge only some cases have been reported in the literature.The purpose of this work is to recall the anatomo-clinical characteristics of this little known entity through a particular observation by the occurrence at an advanced age and by the large size of the lesion.
Methods: We report a new case of GCF of the oral cavity
Results: A 50-year-old woman presented to dental medicine departments with swelling of the oral cavity. Intra oral examination revealed an exophytic pediculated lesion appended to the gingiva with no pain or tenderness. Radiological examination revealed the presence of bone lysis in the bone septum between 35 and 36. The patient underwent an excisional biopsy. Macroscopically, it was a whitish polyp of lobulated appearance measuring 3 cm. On histological examination, the lesion was made up of loose or dense connective tissue composed of fibroblasts and fibrocytes along with many stellate shaped giant fibroblasts sometimes multinucleated. Overlying epithelium was largely ulcerated. Based on the above features a diagnosis of GCF was made.
Conclusions: The GCF is a new entity rarely suspected clinically because it is little known by dentists and because of its similarity with other more frequent hyperplastic lesions of the oral mucosa (papilloma, fibroma or fibroepithelial polyp). It occurs mostly during the first 3 decades of life. Our observation is original by the occurrence of the lesion in the 6th decade and by its size which exceeds by far 10mm (30mm). Surgical excision allows healing because the recurrence is exceptional and allows for a histological analysis that alone can make the diagnosis of FCG.
