Brown tumors of the jaws: a retrospective study

Objectives: The objectives of this study were to assess the various clinical and radiological features of maxillary brown tumors and to discuss their treatment modalities.

Methods: We conducted a retrospective study of brown tumors of the jaws. Data were collected at the maxillofacial and plastic surgery department and at the oral medicine and oral surgery department in Sahloul hospital for a period of 17 years from 2000 until 2017. Clinical, radiological, biological and histological variables and treatment were analyzed. The assessment of the results was done by Excel software.

Results: There were 16 patients who met the inclusion and exclusion criteria. The mean age was 48 years. Brown tumors were associated with primary hyperparathyroidism resulting from parathyroid adenoma in 6 cases (38%) and were related to secondary hyperparathyroidism resulting from chronic kidney failure in 10 cases (62%). Their location was maxillary in 7 cases, mandibular in 5 cases and bimaxillary in 4 cases. A well defined lytic lesion was the most frequent radiological presentation The treatment consisted of parathyroidectomy in 13 patients and maxillary resection in 3 cases. The evolution was favorable with tumor regression in 9 cases and unfavorable in 3 cases.

Conclusions: The diagnosis of brown tumors of the jaws can be difficult since its variable clinical and radiological presentations. An early diagnosis of hyperparathyroidism permits to avoid surgical treatment of these lesions, which would regress after excision of the parathyroid lesion.
