Efficacy of buccal infiltration and intra-septal injection as supplemental technique to the IAN Block in mandibular molars with symptomatic irreversible pulpitis: a comparative clinical study

Objectives: To compare the anesthetic efficacy of IANB plus buccal infiltration (BI) and IANB plus intra septal injection (IS) in patients with symptomatic irreversible pulpitis. We also investigated the correlation between the overall efficacy of anesthesia and factors such as gender, stress level, smoking, alcohol and caffeine consumption previous anesthetic failures, facial type, and presence of pulp stones.
Methods: Sixty-one emergency patients with symptomatic irreversible pulpitis in mandibular molars were divided into two groups and randomly received a conventional IANB with 1.7 4 mL of 4% Articaïne/HCL with 1:100,000 epinephrine plus either BI or IS injections containing 0.4 mL of the same anesthetic solution. Patients recorded their initial spontaneous pain and upon cold testing and when endodontic access was performed, on a Heft Parker visual analog scale (VAS).
Data analyses were performed using the IBM SPSS Statistics ® software version 23.0. Student t test was used to compare age, VAS initial spontaneous pain scores and upon cold testing. Pearson’s chi-squared test was performed to determine differences in gender and success rates. Comparison were considered significant if p < 0.05
Results: 31 men and 29 women, with an average of 35 years ranging from 18 to 71 years. Anesthetic success was obtained in 60% for IANB plus BI (IANB/BI) compared with 43 .3 % for IANB plus IS (IANB/IS). No significant difference was found between the two groups. (P = 0.219 > 0.05).
No correlation could be found between the overall anesthetic success rates and the studied factors
Conclusions: Combining IANB with supplemental buccal infiltration or intra septal injection provides acceptable success rate in mandibular molars with symptomatic irreversible pulpits. Some cases may still need a second supplemental anesthesia or preemptive medication.
