Prevalence and outcome of cervico-facial tuberculosis in Sfax, Tunisia.

Objectives: The aim of the present study was to assess cervico-facial tuberculosis (CFTBC) prevalence, and to investigate clinical, biological and therapeutic outcomes.
Methods: It is a prospective clinical study, involving155 patients diagnosed with (TBC) during 2016, at the anti-tuberculosis center of Sfax, Tunisia. Information regarding demographic, clinical, biological, radiographic and therapeutic features were recorded on a clinical sheet. All oral examinations were performed by the same investigator. Data were analysed using the Epi info7.2 Software.
Results: Patients’ age ranged from 1 to 92 years, with a mean of 39.5±20.9 years and a sex ratio of 0.9. Slightly, more than 60% of them came from rural areas. Consumption of unpasteurized milk or dairy products was found in 12 cases. History of dental care was mentioned in 15.48%. The onset of the disease was progressive in 81.9%. The diagnosis of TBC was made after 1 month for 81.3% of the patients. Constitutional symptoms were found in 100 cases (64.5%), when they existed, fever was predominant (77%). Extra-pulmonary TBC was present in 63.87% followed by pulmonary TBC in 36.13%. CFTBC was found in 45 cases (29%), of these, 42 were primary and involved sub-mandibular lymph nodes. Slightly more than 20% (22.22%) of the patients had poor oral hygiene. The IDR was positive in 68.39 %. The diagnosis was based on histopathology after fine-needle aspiration of lymph nodes (7 cases) or biopsy (38 cases). PCR was positive in 4 cases, direct examination in 3 cases and culture in 3 cases (2 M.tuberculosis and 1 M.bovis). Anti-tuberculous quadritherapy was administered for 2 months followed by a 7-month course of bitherapy. A favorable outcome was achieved in 39 cases (86.66%). Paradoxical response was seen in 2 cases.
Conclusions: The study shows that primary lymph node tuberculosis is still frequent. A multicentric investigation is needed to diagnose primary or secondary oral tuberculosis.
