Prevalence of oral lichen planus and lichenoid lesions in Sjogren's Syndrome Patients: a retrospective study.
Objectives: Our aim is to investigate through a retrospective clinical study the prevalence of oral lichen planus (OLP) and lichenoid lesions (OLL) in Sjogren's syndrome (SS) patients as we want to draw attention to the coexistence of these diseases which have very limited data in the literature. Methods: A retrospective clinical study was conducted at the department of Oral medicine-Oral sugery of SAHLOUL Hospital of Sousse from January 2012 to September 2017. We included patients diagnosed with Sjogren's Sydrome according to the AECG (American-European consensus group) diagnostic criteria. For these patients, we investigated those affected by oral lichen planus (OLP) or lichenoid lesions (OLL) determined according to the WHO (World Health Organisation) classification of 2003. Clinical variables such as age, sex, medical conditions and medications, type of Sjogren's syndrome (primary or secondary), clinical form of OLP and treatment were analysed. The assessment of the results was done by Microsoft Excel software. Results: We evaluated 19 patients (17 females and 2 males) diagnosed with SS (15 had pSS and 4 had sSS) with a mean age of 55,89 years (±11,714). OLP occured in 5 (26.3%) patients (3 with pSS and 2 with sSS) while 1 (5,2%) patient had an erythematous oral lichenoid lesion. OLP was atrophic in 1 patient and erosive in 4 patients, one of whom had the vulvo-vaginal-gingival syndrome. Four patients showed improvement in symptoms on topical treatment while 2 showed improvement only under general treatment with corticosteroids. Conclusions: The results of our analysis suggest that an association may exist between SS and OLP or lichenoid lesions. This leads to the importance of screening for oral dryness in patients with OLP. At present, there is no definitive hypothesis that explains the coexistence of OLP and SS. Further research is required into the mechanisms whereby this coexistence occurs.
