Oral health in Patients with High Risk Cardiac Diseases: a Case-Control Study

Objectives: To assess Oral health in patients suffering from high-risk cardiac conditions.
Methods: This is a case-control study including 60 high-risk cardiac patients (CP) and 60 healthy control patients (HP). Patients were matched for age and gender. Information regarding patients’ characteristics, oral hygiene habits, plaque index, number of teeth present (NT), DMFT scores and tooth mobility (TM) were recorded based on a clinical data sheet. The presence of radiolucent apical lesion and alveolar bone resorption (ABR) were also investigated from panoramic radiographs. Results were reported as mean±SD and percentages. Student t-test, Chi2 test and logistic regression analysis were performed for correlations. The level of significance was set at p<0.05.
Results: The age of studied patients varied from 20 to 83 years with a mean of 49.43±2 years. For both groups, there was no statistical significant difference regarding tooth brushing frequency and the number of visits to the dentist. Although, mean DMFT indices were comparable between the 2 groups (12.1 for CPvs11.7for HP), they were higher in CP in the 20-39 years age group (8vs4.2, p<.05). Indeed, the NT was significantly lower in these patients belonging to the same age group (p=.001). In CP and HP, respectively, the frequency of dental roots (3.05%vs1.45%, p=.002), tooth mobility (11.79%vs6.41%, p=.000) and alveaolar bone resorption (75%vs58.3%, p=0.04) was significantly higher in CP than HP, regardless of age groups. Multivariate regression analysis showed that tooth mobility was the most associated variable to high-risk cardiac condition.
Conclusions: Within the limitations of the present study, patients with high-risk cardiac conditions present lower oral health status than healthy patients.
