Orthodontic treatment and caries risk: Study of salivary changes

Objectives: The aim of this study was to evaluate the effect of orthodontic treatment on physicochemical, biochemical and microbiological salivary properties incriminated in white spot lesion’s development.
Methods: A cohort study was conducted on 61 healthy patients (mean age, 13.3 ± 3 years) followed for both fixed and removable orthodontic treatment. Salivary and supragingival plaque samples were taken at baseline, 1 and 9 months after the orthodontic appliances placement. The saliva buffer is measured according to the Ericsson method.We used the Wilcoxon Test for bound samples -Chi-square test for the bi-varied analysis of qualitative variables and the Pearson correlation test for quantitative variables.
Results: A statistically significant difference in the oral bacterial flora composition between T0, T1 and T2(p=0,003) was revealed as well as for certain salivary biochemical parameters examined such as amylases, lipases, calcium...However the buffer capacity, Ph (p=0.474) and flow rate (p=0.289) were non significantly changed between the three time points and regard less of appliance types.
Conclusions: In this study, the uses of orthodontic appliances have changed certain salivary parameters both after one and nine months compared with the baseline. The orthodontist must consider these changes in the choice of orthodontic appliance’s type and materials in order to prevent the occurrence of white spot lesion by limiting those salivary changes.
