Posterior Edentulism and Temporo-mandibular joint disorders: A Systematic Review

Objectives: The dysfunctions of the manducatory system are complex pathologies with multiple manifestations which makes their diagnoses complicated. Nowadays, it is known that temporomandibular dysfunctions are subject to multiple factors that contribute to their etiology, namely: a structural, psychosocial and a systemic factor. The aim of this systematic review is to study the association between posterior partial edentulism and temporomandibular dysfunction.
Methods: A search of articles on PubMed was done using two Boolean equations: “Jaw, edentulous, partially” [MeSH] AND “Temporomandibular joint disorders” [MeSH] and “Tooth loss” [MeSH] AND “Temporomandibular joint disorders” [MeSH].
Results: From the 42 identified articles, 14 articles were selected. The most common type of articles was cross-sectional studies. The age range of the study population was predominantly between 15 and 40 years. The most common signs of TMD are muscle or joint pain, joint sounds and dyscinesia. In 57% of the articles we did not find any relationship between posterior edentulism and the TMD. In 36% of the articles a positive relationship was found.
Conclusions: Locked occlusion appears to be more pathogenic than having shortened arches in the TMD installation. There is no scientific evidence for the effectiveness of prosthetic restoration in preventing temporomandibular dysfunction. Prosthetic treatment is indicated only after initial treatment of pain resolution and normalization of occlusal functions.
