Cytological Diagnose Carcinomasarcoma of Parotid Gland- A Case Report

Objective: Carcinomasarcoma (malignant mixed tumor) is a biphasic neoplasm composed of malignant epithelial and malignant mesenchymal component. This uncomman neoplasm constituted 0.4% to 0.16% of all salivary gland neoplasms, and 0.4% of all malignant salivary gland neoplasms. Up to date, only more than 60 cases have been reported in English literature.

Methods: Here we reported a 57-year-old man suffered of recurrent enlargement firm mass fixed surrounding the right parotid gland region. Using ultrasound-guided fine neddle aspiration of the lesion and exams with cytological smear method for pathological exams, carcinomasarcoma of the protid gland was confirmed.

Results: The cytological smear consisted of many sheets of single malignant epithelial cells with marked pleomorphic nuclei, increased of nuclear-cytoplasmic ratio, and coarse chromatin pattern. Clusters of chondrosarcomatous, osteosarcomatous cells with nuclear atypia and abundant necrotic debris, and naked nuclear in the smear back ground may help us recongnize the definitive diagnosis of carcinomasarcoma.After total parotidectomy and neck dissection, the histological features also confirmed the diagnosis.

Conclusion:Although histopathologic method was the most common way for disease diagnosis, cytological smear method may also be an effective way for confirm carcinomasarcoma diagosis before surgery.
