Clinical Trial of a Self-etch Adhesive for Preventive Resin Restoration

Objective: To evaluate clinical performance of preventive resin restoration using a total-etch adhesive and a self-etch adhesive in first permanent molars.

Methods: Twenty-eight children with fifty-three matched pairs of permanent molars fulfilled eligible criteria were enrolled. The samples were cleaned, prepared cavity and cotton roll isolated by one operator. They were randomly assigned to control (AdperTM Single Bond 2) or self-etch adhesive (Clearfil SE BondTM) in split mouth, matched pair study design. Prepared cavity were etched, primed and bonded according to clinical steps of each adhesive, restored with resin composite (Z 250) and applied sealant (Concise white sealantTM). Follow-up evaluation was performed by two assessors at 6, 12, 18 and 24 months respectively. Clinical outcomes of preventive resin restoration included sealant retention, resealant need, marginal discoloration and marginal adaptation of restorative material and presence of dental caries.

Results: Total etch adhesive groups showed higher sealant retention rate than self-etch adhesive groups after 12 months (p<0.05). Resealing need of self-etch group was higher than total-etch group after one year (P<0.05). Success of composite restorations was not significantly different between two groups, only one in self-etch adhesive group had margin which explorer can catch. Although, restorations with partial loss and total loss of sealant were not repaired during this study time, there was no caries detected.

Conclusion: Clinical performances of preventive resin restorations were not significantly difference between two groups within one year. At 18 and 24 months, clinical performances of restorations using self-etch adhesive were lower than total-etch adhesive in sealant retention.
