Class III Treatment Concepts: Morphology before and at Long-term Follow-up

Objective: To study the dento-facial morphology in Class III patients with reverse overjet who were treated with dento-facial orthodopedics (DFO) and tooth movement (TM), respectively, at start of treatment (T0) and at long-term follow-up (T2). MATRIAL: DFO group comprised of 21 consecutive patients treated with reverse headgear for 10 months at 9.2 years, followed to 17.4 years (T2). TM-group comprised 25 consecutive patients treated by 2X4 appliance for 7 months at 10.2 years, followed to 16.5 years (T2). There was a positive overjet in all patients after treatment (T1), and at T2 in all (TM and DFO-Stable groups) except 7 patients (DFO-Relapse group). Method: Lateral cephalograms obtained at T0 and T2 were analyzed according to Pancherz (1982). Results: At T0 maxilla was more retruded in DFO group (P<0.05). At T2 maxilla was in similar position (ns) in all groups. Mandible was more protruded and its incisor more retruded in DFO groups (P<0.001). A-Pg difference was larger in DFO-S group (P<0.05) and DFO-R group (P<0.001) than in TM group. Conclusion: 2X4 appliance was a successful treatment concept. Reverse headgear treatment resulted in improved position of the maxilla also in the long-term. In late adolescence its position was similar to that of 2X4 patients. Mandibular growth is not affected by Reverse Headgear in the long-term. In patients with some unfavorable mandibular growth, advancement of the maxilla and dento-alveolar compensation of the mandibular incisors can maintain a positive overjet post-treatment. Mandibular growth pattern is not detectable from cephalomatric readings at young age.
