Factors predicting cervical wear severity in a group of Thais

Objectives: Thailand Oral Health Survey in 1989 reported 8.8% prevalence of cervical wear for the northeast region compared to 4.2% of the nation. Methods: In order to investigate factors that are associated with such high prevalence, 89 dental patients (25 males, 64 females) who attended for dental care at the Faculty of Dentistry, Khon Kaen University volunteered in the study. The patients were examined for the number and severity of both cervical and occlusal wears and interviewed for any behavior that might be associated with cervical wear. In addition, their maximum incisal bite force, salivary properties, tooth-brushing force were measured and a 14-day diet record was collected. Results: The results showed that the numbers of teeth with cervical wear were 4.9±4.8 and 3.6±3.8 in males and females respectively. Both number and severity of cervical wear were significantly higher in males (p<0.01) but these were not true for the occlusal wear. The multiple regression analysis showed that the cervical wear severity was positively correlated with the maximum incisal bite force, age and preference of sweetness but negatively correlated with the frequency of sour food consumption obtained from the diet record (adjusted R2=0.32; p<0.001). On the other hand, the occlusal wear severity was positively correlated with mean occlusal brushing force and age (adjusted R2=0.15; p<0.001). Conclusions: The study confirmed an increased severity of cervical wear with age in previous studies but seemed to discard the risk of incorrect tooth-brushing. It was apparent that the cervical wear severity might be enhanced by a large bite force. Moreover, the occlusal and cervical wear severity seemed to be affected by different risk factors.
