Improving Tooth-Brushing Technique by an IMB Model-Guided Hand-On Oral-Health-Promotion-Program (OHP)

Objectives: This study aimed to evaluate the quality and technique of tooth-brushing among junior high school students according to the IMB model.
Methods: An oral health promotion program (OHP) was provided to the grade 7-8 students of the Santiratwitthayalai School, a public school in Bangkok, Thailand. The students were clustered by their classroom, randomly selected and randomly allocated into the experimental (GroupE-received the OHP) and the comparison (GroupC-received routine curriculum) group. The OHP included 2-hour-lecture-session (focused on knowledge, attitude, and self-efficacy) and a 45-minute-hand-on-session focused on modified Bass technique (MB). The quality and technique of tooth-brushing were repeatedly evaluated at week 0, 1 and 4 by the simplified oral hygiene index (OHI-S) and the observation (placement and movement of toothbrush), respectively. Data were analyzed using descriptive statistics, Chi-square test, and repeated measure ANOVA by SPSS for windows, version 22.0 (IBM Corp.).
Results: 137 students (Female=60, Male=77) were recruited into GroupE (64 students, 13.73(0.75)-year-old) and GroupC (73 students, 13.83(0.63)-year-old). Nearly half of students in both groups had monthly allowance more than 3,000 Baht. Mean OHI-S score at each time point did not differ (F = 1.99, p-value = 0.119). It decreased in GroupE (1.84(0.55), 1.40(0.58), and 1.08(0.61), respectively) and slightly decrease in GroupC (1.76(0.66), 1.54(0.62) and 1.21(0.50), respectively). The effect of group on the OHI-S score was significant (F = 31.79, p-value <0.001). The tooth-brushing technique differed between groups. There was dramatically change in amount of corrected MB areas among GroupE, (F = 31.79, p-value <0.001) and amount of MB-used students in GroupE increased when time passes by (F= 47.244, p-value <0.001).
Conclusions: The OHP which consisted of lecture- and hand-on-session can improve quality and technique of tooth-brushing among the grade 7-8 student.
