The Effective Scaling Strokes on Exposed Root Surface.

Objectives: Periodontal treatment is to achieve sterilization on the exposed root surface. Previous studies reported LPS penetration at exposed root surface and deep scaling is not necessary to remove LPS. LPS is strong immune inducer and is one of the pathogen-associated molecular patterns(PAMPs). Recently it has been reported that PAMPs cause inflammation and immunoreaction, and have an important role in periodontitis progression. However the PAMPs on root surface is unknown. In this study, we investigated the depth of PAMPs penetration on the exposed root surface and the influence of these PAMPs against the cells. Then, we clarify the effective scaling strokes to remove PAMPs on periodontally involved root surfaces.
Methods: Extracted tooth were used for this experiment.After removing plaque with sterilized gauze, exposed root cementum was scaled with sterilized sharp hand-curette. Scaled products were collected for each stroke. Collected samples are crushed by homogenizer and used as stimulants. The presence of the bacterial genome in the sample was identified using PCR method. THP-1 cells were incubated in the presence of the stimulus, and mRNA expression of inflammatory cytokines are measured by real-time PCR. The scaled root surface is scanned with a laser microscope and its depth is measured.
Results: Bacterial genome DNA was detected in all stroke samples. Monocytic THP-1 cells produced Inflammatory cytokine IL-1β stimulated with scaled exposed root surface samples, even with the fifth stroke samples. In laser microscope measuring, the depth of scaling was 20-30μm at the first and second strokes, and about 10μm at the third, fourth and fifth strokes.
Conclusions: This results suggested that bacterial penetration was observed in 80um depth (SRP 5strokes) on the exposed root surface and these infected surface can introduce inflammatory response in Monocytic THP-1 cells.
