Retention and Patient Satisfaction of Locator in Single Implant-Overdenture

Objectives: The purpose of this study is to study the changes of retention and patient satisfaction on mandibular single implant-overdenture (S-IOD) retained by locator attachment, over ten months of use.
Methods: Single symphyseal implants were inserted in twenty patients of S-IOD study project, after that their mandibular complete dentures were modified as S-IOD, retained by locator attachment. The principal operator decided the height of abutment and colour of nylon-insert (14 Blue and 6 Pink inserts) according to existing retention of denture because manufacturer stated that different colours of nylon-insert provide the different amount of retention (Blue 6.67 N, Pink 13.35 N). Subsequently, the retention of each S-IOD was measured by portable force gauge at the baseline, 1st, 2nd, 3rd, 4th, 6th, 8th and 10th months, but some patients were stopped to measure and replaced nylon-insert during 10 months. Patient satisfaction on the retention was assessed by a visual analog scale on monthly follow-up. Retention values and satisfaction scores were analyzed by Steel Test with control and Steel-Dwass Test respectively, after that, correlations between the outcomes were analyzed by Spearman’s rank correlation coefficient.
Results: The median retentive force at baseline is 11.5 N for Blue and 21.9 N for pink. The retentive forces and patient satisfaction of both locators throughout 10 months show no statistical significant difference. Patient satisfaction and retention value have positive correlation (ρ=0.407, P<0.05).
Conclusions: The retention of locator nylon inserts seemed not to change over 10 months of use. The changes of retention is assumed to associate with the condition of alveolar ridge. Although patient satisfaction is proportionately related to the amount of retention, it becomes reverse when the retention is too high.
