Awareness of Dental Implant Care and Its Correlation with Oral Hygiene Practices and Presence of Peri-Implant Disease

Objectives: The importance of professional and self-maintenance in preventing peri-implant disease and ensuring long-term stability has been well established but there is a lack of literature regarding patients’ knowledge and attitude towards implant maintenance. The purpose of this study is to investigate how patients’ knowledge, attitude, and perception influence their oral hygiene practices as well as peri-implant health.
Methods: Participants with single implant crowns placed in 2007-2011 were invited to participate in this study. A total of 79 participants were recruited and all completed the study. Participants completed a survey that examined their knowledge, attitude, and perceptions on oral hygiene practices and peri-implant health before receiving a periodontal examination, prophylaxis, and oral hygiene education. The clinical parameters, peri-implant probing depths, bleeding on probing, plaque index, width of keratinized mucosa, and implant crown characteristics were recorded. The reason for tooth loss and status of peri-implant health were also determined.
Results: Majority of the participants felt that their implants were not giving them problems (78.5%) and were in a healthy state (79.7%). However, only 17.4% had healthy peri-implant tissues with the rest having peri-implant mucositis (73.4%) and peri-implantitis (8.8%). A total of 64.6% of participants did not practice adequate oral hygiene measures and this was reflected in the high mean plaque index of 0.975 detected around the implant crowns. Participants had a median knowledge score of 2/5 when asked about dental implants and its maintenance care. Dental attitudes were significantly associated to age (p=0.032) and ethnicity (p=0.025). Dental perceptions were also significantly associated with age (p=0.023) and education level (p=0.022). No statistically significant association was found between the participants’ knowledge, attitudes, and perceptions and their oral hygiene adequacy, peri-implant health, and plaque index.
Conclusions: Many patients have positive attitude and perception of their implants but do not practice adequate oral hygiene. Clinicians may need to further motivate their patients to perform adequate home care to better maintain good peri-implant health and prevent any bacterial induced inflammatory disease in the peri-implant tissues.
