Clinical and Exfoliative Cytological Evaluation in Somkers and Non-smokers

Objectives: To detect presence of oral mucosal lesion in smoking and non-smoking patients, to compare the CO level between smoking and non-smoking patients and to evaluate and compare the cytology of Papanicolaou (PAP) and Periodic Acid Schiff (PAS) stained cells between smoking and non-smoking patients.
Methods: This is a descriptive cross-sectional study using validated self-administered questionnaires, clinical and exfoliative cytological examination. 80 dental patients were screened for oral cancer risk factors and were clinically examined for oral mucosal lesions. Patients’ carbon monoxide (CO) level was measured by using a portable carbon monoxide detector and the unit recorded is in parts-per-million (ppm) in breath; 0-6ppm (good), 7-10ppm (moderate), 11-20ppm (unhealthy), >20ppm (hazardous). Oral smears were stained using both standard Papanicolaou stain (PAP) and Periodic Acid Schiff (PAS) staining method. All retrieved data were analyzed using SPPS version 22.0.
Results: From 80 patients, 60% are males (3:2) with an average age of 32 years old and of Malay ethnicity (90%). More than half of the patients were smokers (51.2%) and about 7.3% of them had whitish oral mucosal lesions otherwise no oral mucosal lesion was found in non-smoking patients. The mean of CO level among smokers was 7.71 + 4.91 ppm and in non-smokers was 1.49 + 1.64 ppm. Smoking patients had abnormal cellular morphology with positivity to candidal infection whereas in non-smoking patients had normal cellular morphology and with minimal candidal infection.
Conclusions: Only few oral mucosal lesions were noted amongst smokers. Higher CO level and prevalence of candida infection is seen in microflora of smokers. Tobacco smoking induces cellular changes in oral mucosa of smokers compared to non-smokers.
