Effects Of Non-Surgical Periodontal Treatment In Patients With Rheumatoid Arthritis
Objectives: This study aimed to evaluate the effects of non-surgical periodontal treatment in patients with active rheumatoid arthritis (RA) and chronic periodontitis.
Methods: A clinical intervention study was conducted in 82 RA patients with chronic periodontitis. The subjects were randomly divided into 2 groups. Group 1 (Intervention group) received oral hygiene instruction and non-surgical periodontal therapy (n = 41 patients); and group 2 (Control group) received oral hygiene instruction only (n = 41 patients). Erythrocyte Sedimentation Rate (ESR), Anti-Citrullinated Protein Antibodies (ACPAs) in serum, Disease Activity Score base on C-reactive protein (DAS28-CRP) and clinical periodontal indices were recorded monitored at the beginning, 3 months and 6 months later after undergoing periodontal treatment. Independent sample t-test and Paired sample t-test were applied.
Results: There were no statistically significance differences in ESR, ACPAs in serum, DAS28 and periodontal parameters between the 2 groups at baseline. DAS28-CRP, ESR and ACPAs levels in serum significantly reduced after 3 months in comparison with baseline data (p <0.01) in intervention group. The reduction of DAS28-CRP after 3 and 6 months were not found in control group (p>0.05). After 6 months, there were significantly differences in periodontal indices (P<0.01) and DAS28-CRP (P<0.05) between group 1 and group 2. But ACPAs level showed no statistical significance difference between the two groups after 6 months (p>0.05).
Conclusions: Non-surgical periodontal treatment played an important role in improving RA severity thought by significantly reducing ESR, ACPAs levels in serum and DAS28-CRP in patients with RA and chronic periodontitis.
