Predictors Of Improvements In Oral Health-related Quality- of- life Following Endodontic Treatment
Objectives: Oral health related quality of life (OHRQoL) is a multidimensional concept and is influenced by many factors. It has been reported that there are significant improvement in OHRQoL following conventional endodontic treatment but information on what factors influence this is scant. This study aimed to determine key factors associated with changes in OHRQoL following endodontic treatment. Methods: OHRQoL assessments were completed on a consecutive sample of 213 patients pre- and 1-month post- conventional root canal therapy. Association between changes in OHRQoL (OHIP scores) and clinical variables (tooth site, total number of treatment teeth, diagnosis category, primary or retreatment, pain severity after treatment), baseline factors to measure oral health status (DMFT, CPI, with removable denture or not and baseline OHRQoL), also with socio-demographic factors (age, gender, income, educational level) was examined in bivariate and regression analyses. Results: Changes in OHIP scores were evidence following treatment (P<0.001). Results of the bivariate analyses identified that baseline variables such as DMFT, number of teeth requiring endodontic treatment and pain severity after treatment were associated with their improvement of quality of life (P < 0.05). When considering the impact of all variables simultaneously in a multivariable analysis, only the baseline OHIP score emerged as significant factors associated with OHRQoL improvement. Having controlled for socio-demographics, clinical factors related to treatment, patients who had poor OHRQoL (upper quintile OHIP-14 scores) before treatment were almost twice as likely to have OHRQoL improved compared with those patients without poor OHRQoL at baseline (OR = 1.76, 95% CI 1.07- 3.25, P < 0.05). Conclusions: This study identified an improvement of OHRQoL in patients after endodontic treatment over a shortly period of time. Associations exist between patients’ pre-treatment OHRQoL and improvement after treatment independent of other clinical factors. These findings may have implications in identifying who benefits from endodontic treatment and the value of patient centred outcome measures.
