Betel quid chewing and dependence among Paiwan people in Taiwan.
Objectives: Areca nut/betel quid has been reported as an important risk factor of oral cancer and potential dependent. However, there are rarely large-scale studies investigating betel quid dependence. This study aimed to investigate the dependent symptoms according to ICD-10, DSM-IV and DSM-5 diagnostic criteria among an aboriginal population which chewing betel quid is prevalent. Methods: This is a cross-sectional study to investigate the aborigines who live in Ping-Tong County (mainly Paiwan tribe). This study applied multi-stage stratified cluster sampling, the stratification was done on townships in the first stage, then on villages in the second stage, and in proportional to the population of residents in each geographical region. The Betel Quid Dependence Scale (BQDS) was used as a screening tool to estimate the dependent symptoms. Stepwise binary logistic regression and ordinal logistic regression were conducted to determine the predictors of betel quid chewing and dependence. Results: There were 843 participants in this study, 290 of them (34.4%) were current chewers, only 16 (1.9%) were ex-chewers. Among the current chewers, 104 (35.9%) of them showed no dependent symptom, 103(35.5%) and 112(38.6%) met the criteria of ICD-10 and DSM-IV respectively, moreover, 42(14.5%) met the criteria of severe substance use disorder according to DSM-5. After adjusting gender, religion, marital status, and occupation, binary logistic regression revealed that the predictors of chewing habit were lower education(aOR=3.494-8.881), middle age(aOR=2.266-2.855), smoking habit(aOR=2.641) and drinking habit(aOR=3.938). Further analysis using multinomial logistic regression revealed that non-smoker (OR=0.427) and non-drinker (OR=0.315) were less prone to be “severe betel quid use disorder”. Conclusions: Betel quid dependence was prevalent among aboriginal chewers. Poly-use of betel quid, cigarette, or alcohol was also prevalent among chewers. Furthermore, smoking and drinking were associated with the severity of betel quid dependence.
