Indications Of Sedation Need For Oral Surgery In Brunei Darussalam
Objectives: Oral surgery (OS) procedures can be distressful especially to dentally-anxious patients. Conscious sedation can help overcome this. However, it is rarely provided in Brunei Darussalam. Hence the objectives of this survey were to determine the prevalence of dental anxiety and sedation need along with their associated factors, for OS patients in Brunei Darussalam using the Indicator of Sedation Need (IOSN) tool. It was also to measure the validity and reliability of the standardised Malay Modified Dental Anxiety Scale (MMDAS). Methods: A cross-sectional survey using the IOSN tool was conducted on 311 patients who were referred for OS in Brunei Darussalam. The IOSN consisted of 3 components i.e. dental anxiety, medical and behavioral, and treatment complexity rating scales. A reliability test-retest method for the MMDAS was performed on 31 malay-speaking Bruneians in the United Kingdom over a period of 4 weeks prior to the survey. The data was analysed using SPSS version 22 with statistical significance of p<0.05. Results: The study found 50.3% of the sample were in the high and very high anxiety groups. Female, younger age-groups, degree-holders, Class 1 socioeconomic status (SES) and infrequent dental attenders scored significantly higher dental anxiety. The proportion of the sample that required sedation was 52.4% with significantly higher IOSN scores in females and Class 1 SES. The reliability of the MMDAS measured using intraclass correlation coefficient was high (0.857). Cronbach’s alpha was 0.874, indicating high internal consistency. Conclusions: This study showed good evidence of validity and reliability of the standardised MMDAS. The findings suggest high anxiety and sedation need in female and Class 1 SES group. It also confirmed the inverse relationship of dental attendance and age to dental anxiety. Finally, it revealed a high sedation need for OS in the population, suggesting the need of the service to be made readily available in Brunei Darussalam.
