8-Year Clinical Behavior of Adhesively Cemented All-Ceramic Single-Unit Restorations

Objectives: To evaluate the clinical behavior of all-ceramic lithium disilicate (IPS e.max, Ivoclar Vivadent) single-unit restorations luted with Multilink Automix (Ivoclar Vivadent) after a mean observation period of around 8 years.
Methods: 55 single-unit restorations (IPS e.max CAD/-Press) were adhesively luted using Multilink Automix. 33 crowns, 13 partial coverage crowns and 9 inlays were placed by two operators. After a mean observation time of 7.9 years in clinical function 49 restorations could be assessed by selected FDI-criteria (Hickel et al. 2010). Three patients could not be reached any more, one crown fractured because the occlusal minimal thickness was not respected, two teeth were extracted because off vertical root fracture respectively postendodontic failure (6 drop-outs). Of the assessed restorations the longest period in situ was 9 years and 1 month and the shortest was 7 years and 2 month. The FDI-criteria include the evaluation regarding aesthetic, functional and biological properties. Criteria concerning the restorations marginal quality were semi-quantitatively evaluated as % of total margin length (SQUACE method). Statistical analysis was performed using the Kruskal-WallisTest (p=0.05) by means of SPSS 19.00.
Results: After almost eight years most restorations presented an “excellent” to “good” clinical performance. 17% of the total length of all the margins showed slight discoloration (FDA grade 2) and 16% of the margins showed minor irregularities.
Conclusions: After around 8 years of clinical service the IPS e.max restorations, luted with Multilink Automix, showed a good to very good clinical long term performance. The wear of three restorations in one patient who was known for bruxism were graded FDA 3 (sufficient). Due to occlusal adjustments and almost 8 years in function, two restorations got grade 3 for their surface luster at occlusal contact point areas and one restoration for the same property at smooth surfaces. In regard to fracture and retention the results were excellent. Some minor marginal deficiencies were found mostly in inlays (Kuskal-Wallis, p<0.05). Multilink Automix can be regarded as reliable adhesive luting composite for inlays, partial crowns and crowns made of lithium disilicate all-ceramic. This finding is in accordance to the two year data published earlier.
