Effect of Prosthetic Crown Bucco-lingual Width on Peri-implant Biomechanical Stress - A Finite Elements Study
Objectives: This study aims to evaluate an intuitive paradigm stipulating that crown bucco-lingual width reduction may be able to decrease the biomechanical stress on the abutment, implant and peri-implant bone. Methods: Three increasing bucco-lingual width mandibular first molar crowns, an 11.5-millimeter length by 4-millimeter diameter Zimmer® TSV shaped implant, and the corresponding abutment were modelized using computer-aided designed (CAD), To improve the clinical value of this study, occlusal loads were transmited through a newly developed CAD food bolus and simulated by 0.01 second duration, 0.1 millimeter vertical shifting of the antagonist tooth . A finite elements simulation was then processed to evaluate food bolus deformation (%) and biomechanical stress on the abutment, implant and peri-implant bone (MPa) Results: For crown bucco-lingual widths of 5.61 mm, 7.04 mm and 8.45 mm, maximum stress peaks increased proportionally and yielded respectively 2.23 MPa, 3.07 MPa and 3.98 MPa. Increasing bucco-lingual widths also led to the modification of topographic stress distribution and intensities. Indeed, in peri-implant bone, stress was raised on buccal and cervical regions. Mean food bolus deformation also increased by a 1,5x fold. Conclusions: This finite elements simulation demonstrates for the first time a direct correlation between bucco-lingual width and biomechanical stress in a single implant-supported fixed crown model, in the presence of a food bolus.
