"Bioseal" Non-obturation Endodontic Treatment for Periapical Healing - pilot study

Introduction: Occasional cases in literature have reported healing of periapical pathology even without obturation. Recently, maturation of immature, nonvital teeth has been documented by revascularization of pulp. It was hypothesized that even in mature teeth, after thorough disinfection of root canal system, sealing of apical root end with hard tissue barrier may be achieved. The term “Bioseal” is coined to denote apical seal with biological tissues.

Material & Method: Institution ethical clearance and informed consent of the patient was taken. Nine cases presenting with periradicular infection were included in the study. Standard endodontic protocol was followed. When all signs and symptoms of root canal infection subsided, intentional over-instrumentation into periapical area was done to induce mild bleeding and stimulation of various stem cells and growth factors in the apical area. The coronal access was sealed by a suitable permanent restoration at the same sitting. The cases were followed up at 6 weeks, 3, 6 and 9 months.

Results: Nine cases (5 males, 4 females) with mean age of 53 years were treated. Follow up: 9 months- 2 cases, 6 months - 4 cases and 3 months- 3 cases.

All the treated cases documented very good response. Resolution of buccal bone expansion, soft tissue swelling and intra oral sinus and on radiograph, healing of periapical lesion was evident.

Conclusion: It is concluded that periapical healing can be achieved, even in the absence of obturation, if root canal system was thoroughly disinfected and a tight coronal seal was provided. The study is continuing to include more number of cases and longer follow up.
