An efficient orthodontic treatment must aim the best occlusal result in the shortest possible time which requires patient’s full cooperation and compliance. Evaluation of patient cooperation and compliance at 3 stages of orthodontic therapy will help us to determine factors leading to poor or good patient compliance and cooperation

Objectives: To evaluate cooperation and compliance of the patients in leveling and alignment, space closure and finishing stage of the orthodontic mechanotherapy.
Methodology: The sample comprised 90 orthodontic patients of both genders; divided into three groups—initial leveling and alignment stage (30 patients), Space closure and molar correction stage (30 patients) and finishing stage (30 patients). Patient cooperation was determined by Orthodontic Patient Cooperation Scale (OPCS) introduced by Slakter et al 12 and compliance of patient was assessed by clinical compliance evaluation form (CCE) of Richter et al 13 which comprises of four factors i.e. oral hygiene, maintenance of appliance, punctuality of appointments and appliance wear.
Results: Clinical compliance in space closure stage was significantly higher than leveling alignment and finishing stage (p<0.05) . Oral hygiene and appointment punctuality was significantly compromised in finishing stage of orthodontic mechanotherapy with mean SD of 14.1 ± 3.15 and 15.5 ± 6.47 respectively as compared to leveling alignment 17.40 ± 3.38 and 19.5 ± 4.79 and space closure 17.00 ± 3.29 and 18.6 ± 5.40.
Conclusion: Patient after observing improvement with leveling and alignment becomes more compliant in space closure stage. Patient usually suffer from chronic fatigue in finishing stage which decreases compliance and cooperation.
