Evaluation Of Re-mineralisation Potential Of Bioactive Glass Reinforced Orthodontic Adhesive

Objectives: One of the hazards associated with fixed orthodontic treatment is the formation of white spot lesions (WSLs). The objective was to incorporate bioactive glass (BAG) and fluoride-based bioactive glass (F-BAG) in orthodontic adhesive and evaluate their performances to heal white spot lesions (WSL) around orthodontic brackets after being exposed to an in vitro caries challenge.
Methods: The sol-gel derived BAG and F-BAG powders were incorporated (5%.wt-) in Transbond-XT (3M ESPE, USA) adhesives. The obtained BAG and F-BAG were prepared by two different calcium precursors and denoted as BAG-1, BAG-2, F-BAG-1 and F-BAG-2. Human extracted teeth were bonded with various combinations of BAG/F-BAG-Transbond-XT adhesives. Artificial carious lesions were created through pH cycling for 14 days. After treatment and debonding the teeth were analyzed through Scanning Electron Microscopy (SEM) and Fourier Transform Infrared Spectroscopy (FTIR).
Results: The experimental BAG and F-BAG based orthodontic adhesives were successfully prepared and were applied on tooth surface. SEM images revealed that the maximum amount of re-mineralization was on those teeth that displayed cohesive behavior after being bonded with adhesives containing ‘F-BAG prepared with calcium nitrate precursor’ i.e. F-BAG-2. The FTIR analysis of the same group depicted the Si-O-Si bond after undergoing pH cycling. The change in phosphate peak (1026cm-1) intensities were observed after remineralization/demineralization process.
Conclusions: F-BAG-2 (calcium nitrate based) when amalgamated with Transbond-XT was the best possible combination among all the experimental groups for re-mineralizing WSLs during fixed orthodontic treatment. BAG-2 (calcium nitrate based) adhesive turned out to be better than BAG-1 adhesive.
