Association between maternal mental health and early childhood caries in a Primary Healthcare Service in southern Brazil

Objectives: The aim of this study was to verify the association of maternal psychosocial aspects and early childhood caries in one year old children in a Primary Healthcare Service in Porto Alegre, RS, Brazil.


Methods: This is a cross-sectional study nested within a cohort (Cárie Zero) developed at the Community Health Services of Grupo Hospitalar Conceição (CHS/GHC). The sample consisted of mothers and their children born in the years of 2013 and 2014 in the covered area of CHS/GHC and the data were collected between 2014 and 2015. Maternal psychosocial, socioeconomic variables and caries prevalence were collected through clinical examination in children, a socioeconomic questionnaire and the Beck Depression Inventory and the Beck Anxiety Inventory (BDI and BAI). The International Caries Detection and Assessment System (ICDAS) was performed by trained and calibrated examiners (Kappa minimum 0.79 intra-examiner) and for analysis purposes was transformed into the dmfs index modified to include initial lesions.

Results: 309 children had a clinical examination performed and 271 mothers answered the questionnaire and the inventories. The children were between 12 and 15 months of age. Maternal psychosocial variables were individually tested, variables with p<0,20 was included in a multivariate Poisson regression model with robust variance to control confounding factors. There were 11 children with caries, a prevalence of 4.06%. ECC was associated with mother’s age (OR=0.88, IC95%: 0.79-0.99) and moderate to severe maternal depressive symptoms (OR=4,5 IC95%: 1.3-18.3) in the multivariate analysis.
Conclusions: It was concluded that children of young mothers with moderate to severe depressive symptoms had a higher prevalence of caries in the first year of life.
