Dental-health status of institutionalized elderly people in Bogotá, Colombia

Objectives: To describe the dental-health status of institucionalized elderly people in Bogotá, Colombia
Methods: With IRB and informed-consent forms, aged people (≥60-years old) from nursing-homes (Bogotá-Health Secretary) without severe-cognitive disability or mouth-opening limitation and with ≥1 tooth, were clinically assessed at institutions. Surfaces of remaining teeth were examined, without air-drying (epi). Collected data included: number of remainin teeth; partial-dental prosthesis use; Root-caries index: Root-ICDAS-epi caries-experience modified system (R-ICDAS: R-Initial-epi -depth<0.5mm; R-Moderate -depth≥0.5mm-<2mm; R-Extense -depth/extension: ≥2mm; R-Fillings) and Root-Caries-Index caries experience (RCI: R-Sound, R-Caries-D-Moderate/Extensive, R-Fillings). Coronal caries indices: Coronal-Conventional caries experience (C-Conventional-DMF: C-Moderate/C-Extense-caries lesions-D, M, F) and ICDAS-epi system (C-ICDAS-epi, including C-Initial-epi-caries lesions ICDAS-1/2). A 23-item oral-health-related practices questionnaire was conducted
Results: 133 residents (23 institutions) were included (mean age: 81±8.8 years). Most (99.3%) had partial-tooth loss (12±7.5). The prevalence of RCI (DF) was 54.1% and RCI mean of 10.68±18.3 (R-SoundS=15.9±17.1; R-CariesS=1.0±1.9; R-FS=0.4±0.8). Corresponding R-ICDAS prevalence was 55.6% and surfaces mean was 1.5±2.1 (R-Initial-epiS=0.1±0.3). The prevalence and mean of C-Conventional-DMFS were of 100% and 93.7±25.4 (C-Moderate/C-ExtenseS=3.5±9.4 FS=14.7±16.5; MS=75.5±33.9), respectively, and the C-Initial-epiS mean was 0.3±0.8. Almost half (41.3%) used partial-removable bi/uni-maxillary prosthesis, which 38.3% were defective. OH-related practices questionnaire shows that during last three months most of residents did not: use medicines for oral ache (84.7%), change food due-to-prosthesis-problems (78.6%), or experienced dental-health related worries (76.3%). Besides, residents report: last dental visit ≧2-year (80.2%); high sugar-content diet (70.7%); toothbrushing before bed-time (72.5%); toothpaste prosthesis cleaning (64.1%); cleaned prosthesis ≥twice-a-day (48.9%); needed help for prosthesis cleaning (55%); and removed prosthesis before bed-time (19.1%).
Conclusions: Overall dental-health status and oral-health related practices in nursing-home population were unfavorable.
