Antimicrobial photodynamic therapy in primary root canals: evidence-based therapeutic - pilot study.

Objectives: Evaluate the effectiveness of single session endodontic treatment in deciduous teeth, and the use of the aPDT therapy as an adjuvant treatment.
Methods: Ten samples of deciduous teeth with necrotic pulps, from children of both genders aged 3-4 years were selected for this pilot study. The root length was measured with a millimeter rule on an intrabuccal periapical radiograph and confirmed with an apical locator model Endus (Gnatus). The area length for chemical-mechanical instrumentation was obtained by the subtraction of 1 mm from the root length. The samples collected in three times (before/after instrumentation/ after laser application) were cultivated in duplicates so that we could detect the total number of viable bacteria. The cultures were incubated for a 5 days at 37°C in an 85% nitrogen, 10% carbon dioxide and a 5% hydrogen. It was possible to make a visual assessment of the total number of viable bacteria unit-forming colonies (ufc). The results were transformed into log10, and analyzed by the wilcoxon test.
Results: There was a significant reduction of the microbiota from time 1 to times 2 and 3 (p=0.006). However, no significant differences were observed in ufc between times 2 and 3 (p=0.059). The lack of statistical difference in microbial reduction with the use of aPDT therapy may be due to the lower number of colonies that were left to be reduced with the laser. It was observed a small bleed in the apical area of the canal, visualized in the absorbent cone. This could have interfered with the staining of the peritubular dentin, which may have impaired laser penetration into the dentin walls.
Conclusions: Significant reduction of bacterial populations indicates the effectiveness of the instrumentation step, providing root canal obturation in a single session. It is suggested to ensure that periapex has no bleeding to avoid interference in aPDT therapy.
