A Novel Approach for Reduction Genioplasty Using Virtual Surgical Planning, Patient-Specific Cutting Guides and Custom Plating With AI Assisted Simulation.
Objectives: How do AI assist the whole surgical procedure from the data acquisition up to soft tissue prediction and following up, in a stepwise manner in Genioplasty cases. Methods: 22 year old dental student female with a mandibular prognathism, long anterior lower facial hight and high mandibular plane angle,underwent a long orthodontic treatment, camouflaging by extracting Bicusped, levelling, aligning and compensation to restore an optimum overbite and overjet, as a result she ends up with a strong chin, long anterior lower face and slightly chin deviation. treatment plan was curated using clinical examination, cephalometric analysis, and 3D measurements, of course with patient desire and expectations, which include a reduction genioplasty both in AP and vertical components. Cutting guides in assembly manner were fabricated with models to verify a smooth surgical intervention, accurate adaptation and planning.. Patient-Specific Plates adds the benefit of selecting the screw’s size, location and trajectory, avoiding thin bone and undercuts as well as vital structures, especially after a long orthodontic treatment. Piezoelectric device was used as alternative cutting device, a precise delicate corticotomy marks through the guide slits. AISimulation enhanced Patient Collaboration by actively engage in the planning process, discussing preferences and visualizing od the prediction outcomes, ensuring alignment between expectations and reality. Results: Virtual surgical planning (VSP) improves the efficiency of the presurgical work-up and provides an opportunity to illustrate the multidimensional correction at the dental and skeletal level, it also provides preoperative insight into the surgical intervention and the fabrication of cutting guides and templates which help decrease intraoperative surgical inaccuracies. A reduction genioplasty using Virtual Surgical Planning, cutting guide for precise osteotomy with piezosurgery, and Custom Plating to avoid vital structure with AI Assisted Simulation gives a better outcomes than traditional methods Conclusions: Virtual Surgical Planning with AI can be a useful aid to traditional treatment planning by facilitating clinical treatment decision making and providing a visualisation tool for both soft and hard tissue outcome prediction in Orthognathic surgery.
