The impact of retraction cords used in fixed prosthodontics on the gingival margin level in humans: A Pilot study.

Objectives: This clinical study aims to examine the impact of placing a plain retraction cord for different periods of time on the post-operative gingival margin level in humans and its effect on periodontal health and patients’ discomfort.
Methods: A total of 10 participants with an endodontically treated lower first molar were consented and selected following inclusion and exclusion criteria. They were randomly allocated into two groups (n=5), with two different preparation techniques. In the first group (G1) the retraction cord was placed after initial reduction while it was placed after finalizing the preparation in the second group (G2). The gingiva was retracted with the same plain cord (Ultrapak#1). Teeth preparations were done by a single operator and digital intraoral scans were recorded pre-operative, 1 week, and 4 weeks post-operative using Medit i-700 to track the vertical location of the gingival margin as a primary outcome. For the secondary outcome -the impact on periodontal health-, the periodontal parameters; plaque index (PI), gingival index (GI), and probing depth (PD) were also recorded pre-operative, 1week and 4 weeks post-operative by a single-blinded periodontist. The patient's level of discomfort was recorded using the Visual Analogue Scale. Statistical analysis was done using SPSS software and using non-parametric tests.
Results: Time of retraction cord placement differed between both groups; G1 (11min6s±0m48s), and G2 (24min48s± 1min52s). There was no statistically significant difference in all investigated outcomes between both groups. There was a tendency for a slight recession between 1 week (0.054±0.012mm) and 4 weeks (0.062±0.0124mm) which could be considered clinically insignificant, pocket depths remained similar after 4 weeks when compared to 1 week. Patient discomfort was not statistically significant between both groups (P=0.317)
Conclusions: When necessary, leaving a plain retraction cord inside the sulcus for approximately 25 minutes appears to have no negative impact on gingival margin level, periodontal health, or patient comfort.
