Demographic profile, general health and dental status of patients with temporomandibular disorders (TMD)

Objectives: To characterize demographic, general health status and dental parameters as well as caries risk factors in patients with TMD.
This study is part of the Dental Oral Scale (DOS) project, a big data project aimed to measure the extent of dental and oral diseases among
IDF soldiers, and their association with demographics and general health status parameters.
Methods: Data includes demographic, dental and medical records of all patients attending IDF dental clinics in 2014, as well as a control group of patients. These data were compared between general military personnel with a medical record of TMD diagnosis, and a control group with no recorded TMD diagnosis
Results: : There were 2917medical records with a diagnosis of TMD [1499 (51.4%) males and 1418 (48.6%) females]. The mean age was 24.99±7.83 (age range: 17-73). Compared to controls, the following parameters were positively associated with TMD diagnosis : age (OR=1.07, p<0.001), female sex (OR=1.95, p<0.001), academic education (OR=1.20, p=0.009), patients who were born in Asia (OR=1.95) and Former Soviet Union (OR=1.32) compared to Israel (p<0.001), hyperlipidemia (OR=2.09, p<0.001), hypertension (OR=1.90, p<0.001), diabetes (OR=1.77, p=0.037), obesity (OR=2.51, p<0.001), fatty liver (OR=3.55, p<0.001), metabolic syndrome (OR=2.28, p=0.013),smoking (OR=2.58, p<0.001), anemia (OR=10.44, p<0.001), nutritional deficiencies (OR=3.07, p=0.022), anorexia (OR=2.96, p=0.010), periodontitis (OR=1.32, p=0.004), more missing teeth (OR=1.05, p=0.006),less decayed teeth (OR=0.96, p<0.001), self-reported teeth brushing more than once a day (OR=1.10, p= 0.002), less use of acidic drinks (OR=1.27, p<0.001),more general medical visits (17.29 vs.12.10, p<0.001),more dental clinic visits ( 10.1 vs.7.6 ,p<0.001) and more canceled scheduled dental appointments (3.55 vs.3.14, p<0.001).Female gender (p<0.001),increased age (p<0.001), birth country in Asia (p<0.001) and Former Soviet Union (p=0.015), anemia (p<0.001) and teeth brushing more than once a day (p=0.004) retained a statistically significant association with TMD diagnosis in a multivariate Generalized Linear Model
Conclusions:
Health authorities should be aware of the high general and dental morbidity in patients with TMD and refer these patients to evaluation by a dentists and physicians.
